Return Window

Levi Form accepts eligible returns within 30 days from the date the customer receives the order.

Return requests submitted more than 30 days after delivery may not be accepted unless otherwise required by applicable law.

We do not currently offer an extended seasonal return period.

Return Conditions

To be eligible for a return, the item must meet all of the following conditions:

  • The item must be unused and unwashed.
  • The item must remain in its original condition without significant wear, stains, scratches, or other signs of use.
  • The original packaging and included accessories should be returned when applicable.
  • Any original product tags should remain attached when applicable.

We accept eligible returns for buyer's remorse, including a change of mind or an incorrect purchase. We also accept returns for items that arrive damaged, defective, or materially different from the item described in the order.

Non-Returnable Items

The following items are generally not eligible for return:

  • Items that have been used, washed, or show significant signs of use.
  • Items damaged as a result of improper use, handling, or storage by the customer.
  • Custom-made or personalized products.
  • Opened personal-care or hygiene products that cannot reasonably be resold for hygiene reasons.
  • Digital products.
  • Items clearly marked as Final Sale or non-returnable at the time of purchase.
  • Items damaged through normal use, normal wear and tear, or improper customer care.

If an item arrives damaged or has a product defect, please contact us before returning it.

How to Request a Return

To request a return, please follow these steps:

  1. Contact us at service@leviform.com within 30 days of receiving your order.
  2. Provide your order number, purchaser name, and reason for the return.
  3. If the item is damaged or defective, provide clear photos of the item and its packaging.
  4. We will review the request and provide return instructions if the return is approved.
  5. Ship the item according to the return instructions provided by our customer service team.
  6. After the returned item is received and inspected, we will process the applicable refund.

Please do not send a return to a different address without receiving return instructions from our customer service team.

Return Shipping Costs

For returns caused by buyer's remorse, an incorrect purchase, or another reason unrelated to product quality, the customer is responsible for the return shipping cost. Return shipping is charged based on the actual shipping cost from the carrier selected by the customer.

If an item arrives damaged, has a manufacturing defect, or is materially different from the item ordered, please contact us before returning it. Once the issue is confirmed, Levi Form will cover the eligible return shipping cost and may provide a return shipping label or other return arrangements.

Whether the original outbound shipping charge is refunded depends on the reason for the return and the applicable refund terms.

Refund Processing

Returned items will be inspected after they are received.

For approved returns, refunds will generally be processed within 3–5 business days after the returned item has been received and inspected.

Refunds are issued to the original payment method used for the purchase, such as a credit card, debit card, or PayPal.

The time required for the refund to appear in the customer's account depends on the customer's bank, card issuer, or payment provider.

Levi Form does not charge a restocking fee.

Lost or Damaged Return Packages

Customers are responsible for securely packaging returned items and following the return shipping instructions provided by Levi Form.

We recommend keeping the return tracking number and shipping receipt until the return has been confirmed as delivered.

If a return package is lost or significantly damaged during transit, please contact us promptly and provide the order number, return tracking number, and any information provided by the carrier. We will review the situation and assist based on the available shipping information.

Exchanges and Partial Returns

Levi Form primarily provides refunds for eligible returns.

If you would like to exchange an item, please contact us to confirm current inventory and available options. Exchanges are subject to product availability and may not be available for every order.

Eligible orders may be partially returned. For an approved partial return, the refund amount will be based on the eligible items actually returned and accepted after inspection.
